    What is a Sickling Test Test?

    The Sickling test is a diagnostic test used to detect sickle cell disease (SCD) by identifying the presence of abnormal haemoglobin, known as sickle haemoglobin (HbS), in the blood. In SCD, a mutation in the beta-globin gene results in the substitution of valine for glutamic acid at position 6, causing red blood cells (RBCs) to adopt a rigid, crescent or sickle shape. These abnormally shaped cells can block blood vessels, leading to pain and tissue damage. The test uses the solubility method, where HbS is made insoluble in a phosphate buffer with sodium dithionate, causing crystallization and turbidity. This turbidity is then compared to controls to detect HbS. While the test is inexpensive and easy to perform, it has limitations, including false negatives in newborns (due to high hemoglobin F), and in cases of co-inheritance of α-thalassemia or severe anemia. It also cannot differentiate between sickle cell trait (SCT) and sickle cell disease (SCD) and may give false positives in certain conditions like erythrocytosis or marked leukocytosis.
